Fatty Myocardial Foci in Tuberous Sclerosis.

چکیده
T he patient was an asymptomatic 33-year-old woman who had tuberous sclerosis with heart involvement. Her transthoracic echocardiogram showed multiple intramyocardial homogeneous masses in the left ventricle, the largest being 19 × 15 mm in the interventricular septum (Figure 1), and normal biventricular systolic function. Computed tomography showed images in the interventricular septum with a density consistent with fat (Figure 2). The study was complemented by a cardiac magnetic resonance scan that revealed multiple nodular images in the interventricular septum and left ventricle free wall, with high intensity on T1and lower intensity on T2weighted images (Figure 3). There was no delayed enhancement. Tuberous sclerosis is an autosomal dominant disorder characterised by hamartomatous changes in multiple organs, including the heart.1 Cardiac rhabdomyomas are one of the most common manifestations of tuberous sclerosis. The presence of cardiac fatty tumours has also been described in patients with tuberous sclerosis, but their origin was unknown.2 These fatty tumours may be remnants of rhabdomyomas that, according to the literature, are rare in adults.3 The differential diagnosis of a fatty focus is a lipoma.4 Only histology can distinguish between them, but in this case there was no justification for performing a biopsy, since the lesions were not responsible for any symptoms. The presence of well circumscribed fatty myocardial foci in tuberous sclerosis patients should be kept in mind as a differential diagnosis of the more classically described rhabdomyomas. In the future, these fatty foci may help identify patients suspected of having tuberous sclerosis.

Myocardial Fatty Foci in Tuberous Sclerosis Complex: Imaging Findings
Tuberous sclerosis complex (TSC) is a rare autosomal dominant genetic syndrome. The hallmark of the disease is multiple hamartomatous lesions in multiple organ systems. Common cardiac manifestations of TSC are rhabdomyomas, which are a benign tumor of striated muscle. In some patients with TSC, myocardial fatty foci (MFF) deposition has been described with or without the presence of rhabdomyoma...
متن کاملAssociation of Pemphigus Vulgaris and Tuberous Sclerosis: A case report
Pemphigus vulgaris is a blistering autoimmune disease with suprabasal cleft formation, which is the most common autoimmune blistering disease in eastern countries, such as Middle East. Predisposition to pemphigus is linked to genetic factors. Tuberous sclerosis is also a genetic disorder of hamartoma formation in many organs, particularly the skin, brain, eye, kidney and heart. We report ...
متن کاملCardiac Rhabdomyomas and Congenital Hypothyroidism: A Coincidence or Hamartia
Cardiac rhabdomyomas are the most common primary cardiac tumors in children. These tumors are generally asymptomatic, although they may be associated with neonatal tuberous sclerosis complex. Despite the fact that thyroid dysfunction rarely occurs in tuberous sclerosis, papillary adenomas (hamartomas) of the thyroid gland have been reported in a number of autopsies. Herein, we present the case ...
متن کاملTuberous sclerosis with Cardiac Tumor in fetus with Diabetic Mother
Introduction A heart tumor in children is rare and the most primary tumor of the heart is rhabdomyoma. We report a case of cardiac mass diagnosed at 32th weeks of pregnancy while the mother had gestational diabetes Mellitus. Serial echocardiography revealed regression of the tumor; then follow up of the patient confirmed tuberous sclerosis.
